Use this form for notices being served prior to July 1, 2022.
This form is to be used for: (1) any residential or commercial rent that became due between
March 31, 2022, and June 30, 2022; (2) residential rents when the tenant is NOT a
natural person, without any time period limitation; or (3) any rents due under commercial
tenancies, without any time period limitation.
For residential rents where the tenant is a natural person, use: (1) form PRQ-TP-5 for rents due between September 1, 2020, and September 30, 2021; and (2) form PRQ-CRP-3 for rents due between October 1, 2021, and March 31, 2022.
